In order to locate a Davis Vision Plan provider, follow the steps below:

Go to:    www.davisvision.com

Click on "Find a Doctor"

 You will see the "Member Login" screen.

 Enter your social security number as your Member ID or Login Name.

Enter the first five letters of your last name as the Password.

 You will be linked to "Member Menu" where you should click on "Find a Doctor" to locate a Davis Vision Plan provider in you area.
